Nezia Care is a community-based Home and Community-Based Services (HCBS) provider. Our Supported Community Living (SCL) services help each person live as independently as possible while staying connected to the people and places that matter to them.
Support is shaped around the individual: their needs, preferences, goals and approved service plan. Our mission is to enable persons with disabilities to live in and access the resources of the community around them, and to realize their full potential.

We help people develop and keep the skills of running a home: cooking, cleaning, laundry, budgeting for groceries, and keeping a routine that works.

We work closely with each person, their family or guardian, case manager, MCO and interdisciplinary team, so services follow the approved plan and the person's own goals.
How a referral works
Case managers, families and individuals can all start a referral. Here's what happens next.

We review it
We read every detail and reply to the referral source with our response.
Meet and plan
We meet the person and their team to understand goals, needs and preferences.
Support begins
Services start under the approved service plan, and we keep the team updated.

Nezia Care supports people across the Des Moines metropolitan area and surrounding communities. Services take place in each person's home and community. Our office is in West Des Moines.
